Why these questions
This is the survey an investigator or an HR consultancy opens an engagement with, so it has to produce a picture a report can be written from. Psychological safety and everyday respect are asked separately because they come apart: plenty of places are polite and still unsafe. The barriers question is the one that changes what you do next, since "nothing would change" and "I'd worry about my career" call for different fixes. Department and tenure ride along as cohorts, so you can say which team the problem is in without saying who reported it.
