Why these questions
This is the survey an investigator or an HR consultancy opens an engagement with, so it has to produce a picture a report can be written from. Psychological safety and everyday respect are asked separately because they come apart: plenty of places are polite and still unsafe. The barriers question is the one that changes what you do next, since "nothing would change" and "I'd worry about my career" call for different fixes. Add a department or tenure question and tag it as a cohort, and you can say which team the problem is in without saying who reported it.
Results first show at 8 responses
Nobody sees any answers to this poll, you included, until 8 people have responded: the minimum group of 5, plus a few answers that are held back while the survey is open.
