Why these questions
A quarter's leavers in one send is a calendar that identifies people by the week they left. This poll stays open. You send it on a last day, or hang a QR by the exit interview, and nothing is readable until five people have answered, and after that results arrive in groups of three to five. Tenure and department are still on the form, but one-at-a-time dispatch collapses every cut: you only get those breakdowns if you send to a group of five or more at once.
Results first show at 8 responses
Nobody sees any answers to this poll, you included, until 8 people have responded: the minimum group of 5, plus a few answers that are held back while the survey is open. Below that, an individual answer could be traced back to the person who wrote it, so the results stay sealed. Once it closes with the minimum group in, everything shows. Worth knowing before you send it to a team of two.
