Why these questions

Thirty days is late enough to have an opinion and early enough that the gaps are still fixable for the next hire. Every question is one a person inside their probation period can answer honestly only if nobody can trace it back, which is the reason to run this anonymously rather than as a manager one-to-one. The free-text box asks for one change, not an essay, and says plainly not to name individuals.
