Why these questions

Closing a complaint is not the same as fixing what caused it, and the people who know which one happened are the team it happened on. This asks them directly, and it asks about retaliation on purpose, because the fear of it outlasts the process. It ships with a higher floor than the other templates: results stay locked until 10 people answer, since this usually goes to one team and a smaller floor on a small group starts pointing at people. Set that expectation before you field it, not after.
